I’m supprised i have ua level this high but with only a relatively small pain in my ankle. I just had an attack few days ago that lasted 2weeks. whats happening inputs please guys

No meds to lower blood ua, just drinking lots of fruit juices herbal teas eating lots of greens and no more beer. Be prepared for another attack in due course because your dietery restrictions will not make enough difference to get you down below 5.00mg/dL. Bear in mind also that some fruit/juices contain high levels of fructose, which is bad for gout.

July 25, 2011 at 8:10 am #11695odoParticipantAt such high SUA levels, it is likely that dietary restrictions can make a what seems like a big difference at first, and if you're willing to risk having flares, it might be worth seeing how far your level will fall like this. However, dietary restrictions are very difficult to maintain in the long term and it is a case of diminishing returns because diet is not the only factor affecting SUA. Even when taking Allopurinol, if you don't take enough, your SUA will not fall to a safe level.(<5.00)
As a rule of thumb you can probably say that SUA drops by between 1-2mg/dL for every 100mg of Allo. I would say from personal experience that the same applies to diet control, maybe 1-3 mg/dL(maximum) ? more the higher your level, and less the lower it is to start with.
I would also say that anyone with SUA of 8.00+ should definitely be on 300mg Allo for life.

Started with 100mg a day gradually increase dose each week now I’m on 300mg a day and my index finger and palm hurts so bad this morning I took calchicine hoping the pain will lessen but up to now it still hurts so bad it gives me fever. Is this how it should feel when uric acid is desolving?
August 23, 2011 at 10:14 am #11848polecatParticipantbestest,i feel for you.im new to this to.been on allopurinoll for 2 months and it has not been a easy ride,though i am committed to stick it out through the advice of my rheumotoligist.if you dont have a rheumotoligist do everything you can to get a appointment asap.with the increase of allopurinoll and your current sua level you can certainly expect some flares for some time as your sua levels drop.you mentioned you took colchicine,but how much?i am starting to understand what i have to do with pain management.certainly a flare that has you in tears you have to be aggressive weather you take colchicine,nsaids,steroids,home remedies,etc.if you have not already i urge you to use the forums search engine to better understand how to deal with gout,i have spent hours of pain ridden sleepness nights doing just that and am much more informed now in all types of subjects related to gout.for example just type in allopurinoll or colchicine and start reading.there is a abundance of information in past posts.good luck and hope you feel better.

August 24, 2011 at 5:34 am #11855Keith TaylorKeymasterbestest, there is no on-off switch with gout. It is a series of processes that center around uric acid.
The feverishness is a natural part of your immune system fighting uric acid crystals. If they are newly forming crystals that is very bad. If they are dissolving crystals that is good, but you still have to deal with the pain.
At 6.57, you are only just below the crystallization point of uric acid, so you can quite easily be in the worst of worst gout places where crystals are dissolving then reforming as your uric acid naturally changes through the day. The way to stop that is to get the level down to at least 5mg/dL. I have explained elsewhere that my goal is much lower, but that is your minimum. You might also see earlier references where I have said that 6 is low enough. I no longer believe that, as it does not give enough margin to deal with naturally fluctuating uric acid levels.

That is your foundation for fixing gout, but you still have to cope with pain until all old crystals are dissolved. Knowing that you are in control is one of the best forms of pain relief, or at least it is for me. There are thousands of ways to deal with pain, some of which are medical. You need to find what is best for you.

Pain relieving medicines work for me, but I've had to find out what is best. I've been lucky to have found what works through being hospitalized, but if that did not work, I would speak to a pharmacist, then my doctor – in that order.
For gout pain, a combination treatment is always best. Colchicine is the best thing for stopping the pain from getting worse. However, it does nothing to help relieve the pain, so you need something else. Anti-inflammatories make sense, but are not essential. Any type of analgesic will get you out of the immediate pain. Your low uric acid level will lead to a permanent reduction in the frequency and intensity of future episodes, and after a few months, the pain will cease.

Most likely what you are feeling is an acute attack.
Why not consider bumping up the allopurinol, perhaps even to 600 mg and perhaps only temporarily. You want to get out of that 6-7 UA range as soon as possible?which will cause endless attacks. Remember that those WITHOUT gout can escape attacks in that range but once gout has occurred that?range will be forever dangerous.

If you can lower your dosage in the future all the better but perhaps not because you?started with?the highest serum urate I have ever heard of. I'm surprised you didn't crackle while you walked. Know well that allopurinol may have saved your life.

@ zip2play how temporary how many days? will I take alop for 600mg? Or I just have to stay on colchine? Which is a better choice?

I would think something like 3 months with a couple blood tests to ascertain effect on serum urate. Based on the final urate you can then adjust the dosage or not.

Arcoxia (trade name for etoricoxib) is good for reducing inflammation. The pain reduction is usually pretty good, but with a severe attack, may not be enough. Back it up with something that blocks the pain signals. Your pharmacist / doctor / hospital consultant can offer advice. Only you can select which works best for you.
Quickest route, in my experience is ER (UK – A&E). If you are in pain, these are the people who know how to stop it.
Next best is a pharmacist. They are usually in tune with the latest amd best pain-killers. In the UK, their advice is free. Anybody have any views on US pharmacists?

I had my blood tested today and my ua level now is 4.22 and on 600mg of allop 2weeks now hope the worst is over coz having those arcoxia and naproxen sodium makes me feel like a zombie.

I'm delighted your uric acid is finally under control. Your experience with the pain is nasty, but I'm convinced that things can only get better from now on.
I went back to the beginning of the thread to get a feel for what is happening.
You have come a long way in a few months. My heart and soul are willing that the next few months are even better.

It's a hell of a lot?better than it was when?I first started and had tears rolling out of my eyes just walking ten feet to the bathroom. That went on for months.?I never had pain like that. Over time if your like?most of the?people on here all your pain will pass and this will all be just a bad memory. Hang in there it will get better. Good luck.
