As the topic says. I want to be able to spot a possible attack in the future so I was wondering what the “twinges” feel like that everyone discusses. Are they burning, pinching, tensness, bruised feelings or what? I'm still trying to fight my first attack so I'm not sure what the preliminary symptoms of any future attacks for me may be.
August 10, 2009 at 1:36 pm #5274trevParticipantUsually a burning, gnawing sensation [typically on an old site] that comes and goes randomly.Mostly presents in the evening hours.
Occasionally between attacks a very sharp shooting nerve pain in a not too well localised point-
I've never twigged what this is saying – but usually post attack after worst is over.
General pain in old joint sites if stressed too much. Does not usually start an attack.
Alcohol ,or the wrong type of food will do that very well !
I have also associated, lowish level, daily headaches for some days prior to an attack – this one took me ages to rumble.
August 10, 2009 at 8:07 pm #5282zip2playParticipantI get mine only in the morning and only in my right bunion. It is a deep ache that feels painful when weight is applied to that foot. It feels like it is in the precise middle of that large toe joint. It is usually correlatable with stress, beer, or both.
I give it about 10 minutes to abate and then mosey to the medicine chest…and 2 colchicine.

November 13, 2009 at 3:08 am #6517Keith Taylor (GoutPal Admin)ParticipantProphylactic colchicine is a standard treatment during the first few weeks of allopurinol treatment to minimize associated gout flares in New York, and the rest of the world.
It doesn't make sense to take it in this preventative way if you are not on allopurinol (or other uric acid lowering treatment).

I would not like to take them this way myself though , due to the latent toxicity of the drug.
Used in a gout attack, or with AlloP initiation, makes a lot more sense.

I have been on allopurinol almost 20 years. These toe “twiinges” that I referred to all came while on a recent trial couple year regimen of 200 mg. There was also some suspicious knee and shoulder pain too but whether they were gout related is guesswork.
When on 300 or 400 mg. allopurinol, I never had need for any colchicine at all. I;m on 300 mg. now and plan on staying there unless I see any 6+ UA readings, which I doubt.
Yes, colchicine is VERY effective when used at the first sign of an attack…and very little is required, one or two pills. But let an attack run for a couple days and then you need some high dosing to be effective.
